This tracking program not only tracks the patient’s weight and general well being, it will report realtime 24/7 to our staff facilitating contact and communication at all times. You’re only a phone call or email away. The education segment is video and slide based and is automatically delivered as you reach every milestone in your weight loss journey. As is well known with Dr. Ortiz, Dr. Martinez, and their team of innovators, we are working hard on the Smart Phone Application that will concentrate everything in the palm of your hand. We are proud to inform that this application will link up to your smart scale and wrist band and report realtime your progress including metrics of crucial importance like Body Fat Percentage, Total Body Water, Lean Mass and other valuable information which will be analyzed by our team and action immediately taken if needed. La Aduana e Inmigración de México ahora requiere un formulario FMM completado y sellado al cruzar a México. Complete este formulario en línea para el cruce fronterizo terrestre en El Chaparral. Simplemente puede ingresar al Centro de Control de Obesidad en Leona Vicario cuando solicite una dirección.

Por favor use esto enlace para completar el formulario y luego enviar por fax una copia impresa del formulario FMM a la OCC junto con una copia de su pasaporte dentro de las próximas 24 horas.

Debe traer su pasaporte y el formulario FMM impreso, y debe sellar el formulario FMM en la aduana para ingresar a México. México también requiere que mantenga su pasaporte y el formulario FMM sellado con usted en todo momento. No tener el formulario FMM y el pasaporte completos y sellados con usted podría causar problemas durante su estadía en México o se le podría negar la entrada al intentar ingresar a México. Los pacientes con doble nacionalidad también deben traer y enviar a la OCC una copia de su identificación mexicana (pasaporte o IFE) o permiso de residencia en México. Los pacientes no serán admitidos para cirugía sin este formulario completo.

Si su pasaporte o tarjeta de pasaporte vence dentro de los 6 meses, no podrá completar este formulario en línea, renueve su pasaporte y llénelo en línea; de lo contrario, su entrada a México se retrasará mientras lo llena en la Oficina de Aduanas de México. .

El formulario FMM en línea se puede completar dentro de los 30 días posteriores a la fecha de la cirugía, por lo que si está programado con más de 30 días de anticipación, ocúpese de esto una vez que esté dentro de los 30 días posteriores a la fecha de la cirugía.

Estas reglas también se aplican a su (s) acompañante (s). Asegúrese de que tenga un pasaporte válido y complete el Formulario FMM y regrese a OCC con una copia de su pasaporte.
